Müller muscle-conjunctiva resection. Technique for treatment of blepharoptosis

Putterman and Urist describe a technique that resects the Müller muscle and conjunctiva to correct drooping upper eyelids, with satisfactory results in 27 of 28 operated eyelids.

Putterman AM, Urist MJ · Arch Ophthalmol, 1975 · PMID 1156223 · DOI 10.1001/archopht.1975.01010020595007

Method

  • New technique paper: resection of the Müller muscle and conjunctiva.
  • Patient selection test: the eyelid must lift to a cosmetically acceptable level after a drop of 10% phenylephrine hydrochloride in the eye.
  • Only patients who pass this phenylephrine test have the surgery.

Findings

  • Satisfactory results in 27 of 28 operated eyelids.

What it means for a patient

  • This is the origin paper for Müller muscle-conjunctival resection (MMCR), still used today for mild to moderate ptosis.
  • The phenylephrine test described here is still used to select candidates for this surgery over other ptosis techniques.
  • The abstract does not give a measured eyelid height outcome or follow-up length, only "satisfactory" as the result category.

Terms

  • Blepharoptosis: drooping of the upper eyelid.
  • Müller muscle: a small smooth muscle in the upper eyelid that helps hold it open.
  • Phenylephrine test: eye drops that stimulate Müller muscle to preview how much a ptosis surgery could lift the eyelid.
