Eyelid surgery work

  • All eyelid procedures are done under local anesthesia with intravenous sedation ("deep twilight sleep"), as outpatient surgery. Upper eyelid surgery takes 30 to 40 minutes; combined upper and lower takes about one hour.
  • Upper blepharoplasty. A small incision in the natural upper eyelid crease; excess skin is removed, underlying fat recontoured if needed, and a thin strip of muscle may be removed. Closure uses a fine suture hidden under the skin.
  • Lower blepharoplasty, three approaches. Transconjunctival: a laser incision inside the lower eyelid, recontouring of the three fat pads, no stitches. Skin-muscle flap: incision along the lower lash line, skin removal and muscle tightening. Skin flap only: incision just below the lashes with skin removal. Excess lower lid skin may instead be treated with a small skin strip or CO2 laser.
  • Fat is preserved and repositioned into hollow tear troughs, and canthopexy (tendon stabilization) is used to keep the eye shape.
  • The page describes a conservative approach for men that keeps the lower, flatter male brow line.
  • Upper eyelid surgery is often paired with a brow lift; the brows and upper lids are evaluated before surgery.
  • Brow lift. The endoscopic method uses six incisions of about 1 cm hidden in the hairline; a pretrichial incision at the frontal hairline is used for high hairlines (brow lift).

Galleries (Dr. Alex Caniglia's cases): upper eyelid lift, lower eyelid lift, brow lift / forehead lift.

Other procedures

The site lists facelift, deep plane facelift, mini facelift, neck lift, rhinoplasty, ear surgery, buccal fat reduction, fat grafting, facial implants, lip lift, injectables and CO2 and BBL laser treatments (caniglia.com).

Insurance and cost

  • "If drooping upper eyelids impair your vision, insurance may cover the procedure with proper documentation and testing." Lower eyelid surgery is typically considered cosmetic and not covered (eyelid surgery).
  • No prices are published; pricing depends on upper, lower or combined surgery and is given at consultation.

Recovery

  • Little to no discomfort the next day; patients can see the same day.
  • Sutures are removed at seven days; return to work at seven days; makeup after suture removal.
  • Normal activity after ten days; heavy, strenuous activity or exercise after two weeks.
  • Driving: most patients resume within 3 to 5 days.
  • With CO2 laser resurfacing of the lower lid, camouflage makeup waits 10 to 14 days.
