Eyelid surgery work

  • Upper blepharoplasty. Usually under conscious sedation with local anesthesia. Dr. Shire marks the upper eyelid crease, removes the measured excess skin and, if needed, a small portion of the orbicularis oculi muscle and a small amount of fat; sutures close the incision (upper).
  • Lower blepharoplasty. Takes less than an hour per eyelid. Two techniques: transcutaneous (incision just below the lash line, allowing fat removal or repositioning, muscle tightening and skin removal) and transconjunctival (incision inside the lower eyelid for fat only, no visible scar) (lower).
  • Fat repositioning. Lower lid fat can be repositioned for patients with a hollow look beneath the eyes (blepharoplasty).
  • Brow lift. Performed under mild sedation and local anesthesia. Dr. Shire offers six brow lift procedures. The site describes the Minimal Incision Brow Lift (MIBL), also called Botox Assisted Brow Elevation (BABE), which uses Botox to help position the brows, needs three small incisions above the hairline and no fixation screws, and takes 30 to 40 minutes (source).

The blepharoplasty gallery shows 12 before-and-after image sets of upper, lower and combined eyelid surgery (source). The brow lift gallery shows 9 image sets, some combined with upper and lower blepharoplasty (source).

Other procedures

The site lists facelift, revision facelift, mini facelift, neck lift, rhinoplasty including ethnic and revision rhinoplasty, chin augmentation, otoplasty, lip lift, deep chemical peels with phenol-croton oil, laser resurfacing, and injectables (source).

Insurance and cost

The site publishes no practice prices and no statement on insurance or Medicare coverage for eyelid surgery. The upper blepharoplasty page cites a United States average surgeon's fee and lists facility, anesthesia and medication costs as separate parts of the total.

Recovery

  • Upper blepharoplasty: most patients return to work and daily activities within five days; bruising gone by three weeks; swelling gone after two months; final outcome in four to six weeks at minimum; crease scar fades over 6 to 12 months (upper).
  • Lower blepharoplasty: recovery generally about 14 days; sutures typically removed after a few days, or dissolvable sutures are used (lower).
  • Brow lift (MIBL): most patients return to their normal routine within a week; strenuous activity avoided for about a month; no dressings, bandages or drains (source).
