Medicare and private insurers pay when sagging upper-lid skin or a low lid blocks the top of your vision. A visual field test and photographs show the loss before the operation is approved. In the Honolulu area, Medicare claims go to Noridian Healthcare Solutions, LLC, whose rule is L34194.
