When insurance pays in the Salt Lake City area

The rule
Medicare and private insurers pay when sagging upper-lid skin or a low lid blocks the top of your vision. A visual field test and photographs show the loss before the operation is approved. In the Salt Lake City area, Medicare claims go to Noridian Healthcare Solutions, LLC, whose rule is L34194.
What it pays
Medicare's average payment to the surgeon in the Salt Lake City area is $679 for an upper blepharoplasty and $792 for a ptosis repair (2024). The surgery center or hospital is paid separately. You pay your deductible and coinsurance; any cosmetic part of the same operation is billed to you.
